(Medical Director) Form Reporting [EMS]

Form Reporting is run by selecting a source, form type, and a form from the list. You can also filter the results by choosing a student, preceptor, start date, or end date.

Select a form and any filter results desired, then click the Run Report Summary, or Run Report Detail button.
