The built-in surveys included in the documentation settings can be found on the Settings tab for your Clinical or Field Internship Capstone Classes. Whether it is enabled determines whether students and preceptors will see our default affective evaluation forms as part of the documentation for the student-generated clinicals associated with that class.

C. How to Turn the setting On/Off
A. Selecting Yes:
a. For Students:

When students complete their documentation for the opportunities they have created for that class, they will have the Affective Site/Preceptor Evaluation form on their Forms tab.

b. For Preceptors:

Preceptors will have the Affective Student Evaluation form to fill out on their Forms tab after the student has submitted the documentation to them.
- Responses to these forms are displayed on the Affective Site Evaluation Report and the Affective Student Evaluation Report.
B. Selecting No:
Students and preceptors will not have the default affective evaluation forms associated with the documentation for clinicals created by students enrolled in this class.
Note: This setting will not affect the 'Include built-in surveys during documentation' setting determined when creating opportunities as the instructor. That setting will affect that specific batch, whereas the setting on the class tab will affect only the opportunities created by the student.
C. How to Turn On/Off
1. Begin in your admin role.
Note: Cohort admins can also do this through the dashboard by editing the cohort and navigating to the class.
2. Under the Cohort/Classes/Sites tab, select Manage Cohort Classes.
3. Locate the class and click Edit under Actions.
4. Turn the setting
5. Save.
